What’s the deal with IUD insertion (intrauterine device)?

Many women have questions about IUDs, IUD insertion, and the potential risks of this form of birth control. IUD insertion has risks.

IUD: The Good, the Bad, and the Risks You Should Know

An intrauterine device (IUD) is a small device placed inside the uterus to prevent pregnancy. There are different IUD types: hormonal (like the brand Mirena),1 and a copper IUD (like Paragard).2 A hormonal IUD releases hormones into the uterus,1 and a copper IUD causes inflammation in the uterus that makes a toxic environment for eggs and sperm.2  It’s important to understand that an intrauterine device can have side effects. Like other medical procedures, there are potential risks — both during IUD insertion and afterwards. And women deserve to know about these risks during their decision-making process. Here are some things to know:

During IUD Insertion: The insertion process can be uncomfortable (or even painful) for some women.1 Women may cramp, feel dizzy or faint during the procedure.1 The IUD can pose a risk of uterine perforation (yep, that’s when the device punctures the uterus or cervix).2 Infection is also possible during or right after insertion, which, if left untreated, could lead to pelvic inflammatory disease (PID).3 These risks are definitely something to consider.

After Insertion Risks: Once the IUD insertion procedure is finished, there’s a risk of expulsion (where the IUD moves, falls out, or a cramp pushes it out). A woman might not know this has happened.2 If she becomes pregnant with an IUD, there’s also a higher risk of an ectopic pregnancy (where the fertilized egg implants outside the uterus). That’s a condition that needs immediate attention.4 If someone thinks they might be pregnant with an IUD, it’s vital to talk with the doctor who inserted the device. And also, some IUDs are linked to bleeding between menstrual periods, ovarian cysts, cancer, heart attack, and stroke.5

Knowing these risks helps a woman decide if IUD insertion is the right choice for her. When it comes to any medical decision, it’s your body, your decision. You should feel empowered to ask questions and make the best decision for yourself.
